CERTIFICATIONS PROCEDURES

In order to be considered eligible for the ETCO Certification , a candidate MUST HAVE FULFILLED ALL THE FOLLOWING CRITERIA:

Complete and present:

1. The Application ( Form 0 ) and Payment ( Form 1 ) forms
   
   
2. Certification of present job as a Transplant Coordinator, which has to be signed by the Director of the officially recognized Hospital or local, regional or national sharing office (Should be send in English - Fill Form 2 ) . A candidate must have completed 12 months of activity before application.
   
   
3. The Certification of experience given by the Medical Director of the officially recognized Hospital or by the director of the Local, Regional or National Sharing Office (Fill in the ad hoc document - Form 3 )
   
   
4. A photocopy of the current health professional license (in the native language)
   
   
5. Detailed Curriculum Vitae with specific and continuous relevant training, working experience and publications in the field of Transplant Coordination during the last 5 years (Should be send in English)
   
   
6. A photocopy of the certification or diploma that accredited education and training in the field of Transplant Coordination. It could be by means of any of the existing Courses or Programs of Continuous Education given by Universities, National or Regional Transplant Organizations or by any officially recognized Organ Exchange Organization, or others.
